Summary: MemorialCare Medical Group isseeking to add an outstanding Nurse Practitioner to its expanding WHOLE LifeProgram in the central Orange County area.WHOLE Life provides care to patients experiencing complex healthchallenges who are currently unable to visit their primary care physician(PCP). The goals of the program includetimely, focused care to improve patient health, quality of life, and theability to follow-up with a primary care physician in the clinic setting. This home visit program iscoordinated on behalf of the patient's PCP. The WHOLE Life Nurse Practitionerwill provide frequent updates to the patient's care team and generate specialtycare referrals as needed. The Nurse Practitioner will provideindividualized, compassionate care on behalf of the PCP in the home setting,board and care, nursing homes, or assisted living facilities. EssentialFunctions and Responsibilities of the Job In this full-timerole, you'll be working independentlyto manage and coordinate medically complex patients with regards to thefollowing: Providehistory and physical as appropriate for each patient Effectivelyidentify, evaluate, and address acute health conditions as well as diseaseprevention and health promotion issues Abilityto perform many procedures, including wound closure, I&D, foreign bodyremoval, etc. Maintainslegible, accurate and confidential medical records. Documents all medicalevaluations, diagnoses, procedures, treatments, outcomes, education, referralsand consultation Refer,as necessary certain medically necessary non-emergent hospital and specialtyservices Coordinateand direct appropriate care for patient by means of an initial diagnosis andtreatment, obtaining second opinions as necessary and consultation withcontracted specialists Follow-upon referrals made to specialists to assess the results of the care, medicationregimen and special treatment and ensure continuous care Establishprocedures to contact patients when they miss appointments, requirere-scheduling for additional visits, or confirming referrals to a specialistfor care Coordinatepatient referrals for other service Submitservices with the usual and customary billed charges in a timely fashion Responsiblefor a mental health assessment, ruling out mental disorders due to a generalmedical condition, ruling out substance related disorders and identifyinggeneral medical conditions that cause or exacerbate psychological symptoms Establish and maintain effectiverelationships with members and their caregivers in collaboration with clinicalstaff, PCP's, specialists, Social Workers, Case Managers, etc. Identify the appropriate level ofcare, i.e., hospitalization, home health, palliative care or hospice. Participate in clinical andoperational data collection to demonstrate the impact of the program.
Experience/Qualifications/Certifications: 2-3years NP experience in acute care, primary care or urgent care. Master's Degree in Nursing (required) Active RN license in the State of California(required) Current BLS certification (required) Current Nurse Practitioner certification in the Stateof California (required) Current DEA license Bilingual in Spanish+
