The Manchester VAMC is recruiting for a Home Based Primary Care Director. He/she is accountable for the management, supervision and evaluation of care for patients enrolled in the HBPC and other VA staffed in-home programs. The selected candidate may be a PA, RN, Psychologist, Social Worker, Pharmacist, OT or Dietitian.
