This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Learn more The VA Southern Nevada Healthcare System is currently recruiting for three Senior Social Workers. There is an opening as a Transitional Care Coordinator in the Behavioral Health Service line, a Suicide Prevention Coordinator in Behavioral Health Service, and a HUD/VASH Senior Social Worker in the Social Work Service Line. Behavioral Health Transitional Care Coordinator The primary function of the Behavioral Health Transitional Care Coordinator in the Southwest Primary Care Clinic will be to provide comprehensive case management and discharge planning to Veterans currently receiving services that is managed/monitored by community mental health/medical facilities. Responsibilities include the management, coordination, and provision of social work and behavioral health services to Veterans and their families in community settings. Additional functions include, but are not limited to: - Work in partnership with the Behavioral Health Transitional Care Coordinator RN. - Conduct psychosocial assessments to determine needs of Veterans and family members. - Conduct psychiatric assessments, to include risk assessments when appropriate. - Actively participate in the treatment planning process with other disciplines and assist with discharge planning. - Provide individual, family, and group counseling as appropriate. - Face-to-face contact to introduce the Veteran to the "face" of VA Behavioral Health Service while they are hospitalized at the non-VA inpatient mental health facility. The goal being to optimize continuity of care from the non-VA inpatient mental health facility to the VA outpatient setting. This position requires a high amount of drive time as interventions occur primarily in the community and multiple visits per week may be made to the Veteran as needed. The Behavioral Health Transitional Care Coordinator requires a current valid and unrestricted driver's license. Suicide Prevention Coordinator The Senior Social Worker in the Veterans Recovery Center is responsible for assessing the risk of suicide in individual patients in conjunction with the treating clinician and managing the Category II Patient Record Flag for patients at risk for suicide. Tracks high-risk veterans and monitors treatment to ensure that needed services are being received. Provide appropriate clinical referrals to Veterans affected by a suicide attempt or bereaved by suicide, including trauma treatment and care for complicated grief. Responds to consults from the Veteran Crisis Line within 24 business hours and coordinates the follow-up care for those Veterans and families to assure timely access to care and follow-up for patients in crisis. Provides training to staff at the VA Medical Center, Primary Care Clinics, CBOC's, and community agencies regarding suicide risk factors, warning signs, treatment options, medical center memorandum and standard operating procedures, and resources. Develops and promotes educational materials about the warning signs for Veteran suicide and how to connect individuals in crisis with assistance and care. Works to reduce stigma associated with suicidal behavior and mental health and substance use disorders among Veterans. Promotes the understanding that recovery from mental health and substance use disorders is real and possible for all Veterans. Encourages collaboration between providers of mental health and substance use services and community-based programs, including peer support programs and Veterans Service Organizations (VSO). HUD/VASH Senior Social Worker This position provides field-based intensive case management and intervention support to homeless Veterans in the HUD-VASH program in a shared, team-oriented, recovery-based program. The Senior Social Worker independently provides clinical psychosocial treatment and ongoing case management services at an advanced practice level to eligible Veterans and their family members in their home environment to meet biological, psychological, social and environmental needs in obtaining and maintaining subsidized permanent housing. The primary goal of the program is to end the cycle of homelessness for Veterans through intensive case management and clinical intervention. Some duties include, but are not limited to: Referrals to internal/external stakeholders, such as the Public Housing Authority Screening and psychosocial assessment to determine needs of the patient and family Treatment planning Completing Comprehensive Suicide Risk Assessments and Safety Plans Intensive case management with contact frequency from 1x week to 1x quarter Provide individual counseling using evidenced-based practices such as Motivational Interviewing and Harm Reduction Housing Searches & transportation The HUD/VASH Senior Social Worker requires a current valid unrestricted driver's license. Telework, if approved, would be on an ADHOC basis for HUD-VASH only. Work Schedule: Monday through Friday 7:30 to 4:00 PM. Financial Disclosure Report: Not required
