The Extended Care Advanced Practice Provider under general supervision and in accordance with APP standardized procedures/ protocol: Performs physical examinations and treats common complications of cancer treatment, counsels' patients and families and monitors patient progress. Responsible for acute clinical management of the patient during their visits to extend care. Works in collaboration with the patient's referring provider as well as other members of the patient's care team to ensure optimal patient experience and outcomes. Responsibilities include: Ensure patient information, visit history, refer outs, lab follow-up, phone calls, and consults are carefully and completely documented in the electronic medical record.
