This is a developmental GS-9/11 Clinical Social Worker position assigned to Social Work Service and performs social work duties in the Primary Care Clinics at the Mann-Grandstaff VA Medical Center. The incumbent is on the multidisciplinary Cancer Care Navigator Team (CCNT) supporting cancer care activities across the continuum of care through survivorship/surveillance or end of life care for patients receiving cancer care in coordination with partner VISN 20 facilities.
