Position Summary:Social Work Case Manager will be working with Child Abuse Response Team and Healthy Beginnings Clinic. Primary responsibilities will include clinical assessments, crisis intervention and working with support groups for patients and familyManage the medically related social and emotional needs of patients and their families as needs impinge on patient medical conditions, treatment, recovery and safe transition from one level of care to another.
