Your day.

As a Licensed Clinical Social Worker, you need to know how to:

• Provide triage, assessment, and counseling for Seton Womenâ™s Center patients as identified as needing behavioral health intervention


• Conduct psychosocial assessment interviews with patient and their families and collateral caregivers


• Develop and implement treatment plan for stabilization and behavioral modification, mobilization and disposition appropriate for patient care


• Function as part of a multidisciplinary team to develop patient care plans


• Document care management activities and clinical assessments in patientâ™s medical records as required


• Provide services using accepted psychotherapeutic models and interventions.


• Provide consultation to primary care providers as needed and collaborate with staff and providers on treatment plans.


• Ensure quality of care for patients by maintaining clinical focus on the restoration of the patient to physical and psychological safety.


• Consult and act as liaison with all other members of the treatment team to mobilize patient.


• Provide educational information to patients and their families.


• Provide patients with information regarding community resources and facilitate connections with these resources.


• Provide aftercare planning

Your experience.

• Masterâ™s Degree in Social Work


• Licensed Clinical Social Worker in the State of Colorado  (LCSW)


• Knowledge and experience with managed care

Preferred:

• Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals


• Experience with PC software applications in word processing, Epic, Google applications
