The social worker provides for the needs of the PACT team, the high-risk intensive program and the Post 9/11 M2VA Case Management program. The PACT High Risk Intensive Program is designed to meet the needs of high-risk Veterans who require intensive case management to prevent hospitalization and to coordinate care. The overall objective of the program is to assist the Veteran/caregiver in managing the disease(s) process, coordinating care and maintaining and/or improving health.
