The Patient Aligned Care Team (PACT) Social Worker will provide social work service in Primary Care in a Community Based Outpatient Clinic. Primary duties include completing thorough psychosocial assessments, discharge planning, community and VA referrals, and involvement with interdisciplinary treatment team. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
