Home Based Primary Care (HBPC) is a program offered through the Veterans Administration (VA) under the Geriatrics/Long Term Care Line. The purpose of HBPC is to provide direct care to enrolled veteran patients who are housebound or terminally ill, within the milieu of their home, through an interdisciplinary team approach composed of physicians, nurse practitioners, nurses, social workers, kinesiotherapist, dietitian, chaplain and consultants for clinical pharmacy and geropsychiatry.
