Transplant Social Workers are assigned to the Kidney and Liver Transplant Program and provide continuity of care services for Veterans in outpatient specialty clinics who are referred to the Transplant Program, for Veterans in the Transplant Program who are admitted to inpatient surgical care teams in the hospital or transferred to the Portland Outpatient Procedure and Surgery (POPS) unit following operative care procedures.
